Medvedev led Shapovalov 3-2 in their head to head before the final, the last three going to Medvedev.
The most recent win was in the round of sixteen at Cincinnati this year.
In set one, Shapovalov saved a break point in game two, then broke Medvedev in the following game.
The pair exchanged service breaks in games five and six, leaving the Canadian ahead 4-2.
No further breaks in the set, and Shapovalov served it out 6-4.
In set two, Medvedev saved a break point in the third game, then broke for a 3-1 lead.
He saved four break points in a six deuce game before consolidating at 4-1.
Shapovalov broke back in the seventh game, but Medvedev won the final two games of the set to level at 4-6 6-3.
Medvedev broke Shapovalov to love in the third game of the decider.
He consolidated for 3-1.
Shapovalov was broken again in the fifth game.
He served at 1-5 to stay in the match, and saved three match points, eventually holding for 2-5.
Medvedev raced to 40-0 and had three match points.
Shapovalov fought off all three, but Medvedev won with his fourth to claim the title 4-6 6-3 6-2.
Title number fifteen, and second for 2022.
