Medvedev won all four previous contests between the players, the most recent a quarter final in Vienna last year.
Sinner served first and was taken to deuce.twice before holding.
Serving at 1-2, Medvedev was broken.
Sinner led 40-15 when attempting to consolidate, but Medvedev won the next three points to gain a break chance.
Sinner saved it and held for 4-1.
Medvedev broke back in the seventh game.
Medvedev served at 4-5 to stay in the set.
He held to love.
However, when Medvedev served at 5-6 he was broken and Sinner took the set 7-5.
In set two, Medvedev broke in the opening game.
When serving at 1-3, Sinner saved three break points, but failed to save a fourth and Medvedev extended his lead to 4-1.
Recovering from 0-40, Medvedev held serve to maintain the double break and lead 5-1.
Sinner held serve, forcing Medvedev to serve for the set.
Medvedev held to love and the set was his 6-2.
In the decider, Medvedev broke Sinner in game three.
He then saved a break point before holding for 3-1.
A further service break gave Medvedev a 5-2 lead and he served out the match.
The 5-7 6-2 6-2 win gave Daniil Medvedev his 16th title and first for the year.
It propelled him back to the top ten.
