Djokovic had the early advantage, breaking for a 2-0 lead - Nadal struck back immediately, and games were on serve until 5-5, when Djokovic wasted a game point before dropping his serve.
Nadal converted his second set point to lead 7-5.
Djokovic dominated set two and levelled the match at 5-7 6-1.
In the decider, Djokovic failed to take advantage of break points in the fifth game, and Nadal broke in the next.
It was enough for him to eventually serve it out 7-5 1-6 6-3, for Italian Open number 10.
