The pair had met twice previously - one win apiece, but both matches in 2020.
Paul won a second round in the Australian Open and Dimitrov won an opening round in the US Open.
In this final, Paul broke serve in game two of set one.
He saved two break points in game six before holding for 4-2.
Two breaks of serve followed, leaving Paul ahead 5-3.
Serving for the set, Paul was broken, and led 5-4.
Serving to stay in the set, Dimitrov was broken, and Paul took the set 6-4.
Dimitrov saved two break points in game four of set two and held for 2-2.
Paul broke serve in game eight to lead 5-3.
He served out the match 6-4 6-3.
Title number four, and first since Queens Club in June this year.
